GHC-SCW members may change their addresses online. You need to register online in order to change your address.

Please note the following:

Your PERMANENT address is the subscriber's mailing address to which all health plan and insurance information is mailed. If the subscriber does not live at your household/permanent address, you may not change this address for the household. However, you may change individual dependent's permanent addresses. For example, you may wish to change the permanent address for a daughter at college. Please indicate this on the following form. Your ALTERNATE ADDRESSES are addresses you may use for shipping your online orders, such as prescription refills or contact lenses order. For example, if you prefer to receive a shipment at your office, or perhaps a prescription when you are wintering in Florida, you would change your Alternate Address on this website. The alternate addresses apply ONLY to your online transactions and are not stored in your permanent file.
